Early Childhood Care Coordination
Introducing the Early Childhood Care Coordination (ECCC) program in Okaloosa County – a specialized program designed for children ages 0-5 years old who present behavioral challenges in childcare settings or at home. We aim to strengthen the bond between caregiver and child and promote healthy early childhood neurodevelopment in a safe and stable environment.
Our Early Childhood Care Coordinator (ECCC) is a highly trained professional who will facilitate community integration and continuity of care through multi-disciplinary staffing. We ensure families have linkages to their community and support systems, providing recommendations for interventions in the home setting and linking them to community services as appropriate.
Our ECCC facilitator will identify key stakeholders involved with referred families, determine what services are needed to strengthen the family unit and minimize dependency system involvement up to and including out-of-home placements. We use telecommunication platforms to optimize care coordination services, facilitate case staffing with applicable entities, and identify gaps while assisting in coordinating the alignment of services.
We demonstrate compassionate engagement while working with families and establish working relationships with the Early Learning Coalitions, child welfare workers, Primary care providers, Childcare Centers, and Head Start Programs. We identify and develop relationships with key stakeholders involved with referred families.
