Adult Care Coordination

Adult Care Coordination (ACC) works with eligible adults to ensure they receive the care they need to improve their physical, behavioral, social, and health.

The ACC facilitates connections between healthcare and service providers to support individuals and bridge gaps in the care system. The ACC will connect the adult to a treatment home as well as community support. 

The ACC program serves adults aged 18 and up in Okaloosa and Walton Counties. These adults are high utilizers of behavioral health services.

The ACC facilitates services between clients, family members, and service providers. Clients are referred by community partners such as the Department of Children and Families (DCF), Law Enforcement Officers, Acute Care Facilities, and Hospitals.

The program’s focus is to engage adults, who have 3 or more acute involuntary commitments (Baker Act) within 180 days, who are involved with the Department of Children and Families, Criminal Justice / Law Enforcement, who have a Substance Use Disorder, Mental Health Diagnosis or recently released from a psychiatric hospital. 

The ACC will partner with the individual to facilitate communication between all providers and supports to ensure coordination of care that appropriately meets the needs of the adult.

ACC has relationships with Okaloosa County community organizations and is continuing to cultivate new relationships in surrounding areas.

ACC is a state-funded program. Adults who are uninsured or underinsured may be admitted or referred to the program. 

Natural supports are vital to an individual’s success. The ACC will work to engage these supports in a collaborative effort to ensure the Individuals’ long-term success.

ACC is an intensive 90-day service.
