Position Summary:
Bridgeway Center, Inc. (BCI) seeks applicants who want to work with clients to effectively address clients’ care by assessing needs, developing, monitoring, and facilitating interdisciplinary approaches. This position is responsible for managing an assigned caseload of clients having mental illness diagnoses and for assessing client needs, developing, implementing, and reviewing service plans, and working with other community resources in meeting/achieving client service needs. The Care Coordination Program (CCP) provides services that help individuals link to needed medical, social, education, psychiatric, psychological, and other services. This includes direct assistance in accessing services, Coordination of Care (CoC), oversight of the entire case, and linkage to appropriate services. Care Coordination includes (1) assessment of the eligible client to determine service needs; (2) development of an individualized integrated service plan; (3) referral and related activities to help the client obtain needed services; (4) monitoring and follow-up; and (5) evaluation. The CCP educates persons served and their support system regarding diagnosis, treatment, and recovery. This position will work with clients in the Judicial system.

Minimum Requirements:
- Bachelor’s degree from an accredited college or university with a major in Social Work, Psychology, Counseling, or a related human services field.
Bilingual/bicultural English and Spanish preferred (not required).
Applicants must be able to pass an Agency for Health Care Administration (AHCA) Medicaid and Florida Department of Children and Families (DCF) Level II background check (Clearinghouse Education and Awareness Website: https://info.flclearinghouse.com), National Sex Offender Registry search, Office of Inspector General (OIG) check (if you have previously worked for DCF (including Contract or Subcontract Provider), drug screen, and Tuberculosis (TB) test. Applicants must be at least twenty-one (21) years old and possess a valid Florida Driver’s License with no more than three (3) points on their driving record to be insured under BCI’s automobile insurance policy.

Duties / Responsibilities:
Some of the job tasks and responsibilities include, but are not limited to:
- Accept referrals from child welfare professionals, DJJ, schools, law enforcement, Crisis Stabilization Units, Inpatient Behavioral Health Units and community agencies
- Respond promptly to referrals and facilitate case staffing’s within one workday.
- The CC will maintain at least three client contacts and or attempts within the initial 30 days of service.
- If the individual served is not responding to attempted contacts, the CC must document this in the client record and make active attempts to locate and engage the individual in the community.
- Appropriate ROIs are completed for the community partners.
- In partnership with the person being served and family, completes the CC Initial Assessment and Initial Service plan.
- Initial care plan is collaboratively completed within seven days of initiation of services.
- In cases of youth served, should the family desire to seek residential placement, the CC will actively assist the family in completing the SIPP Staffing request application. The CC will scan necessary documents (i.e. birth certificate, social security card, etc.).
- In the cases of youth served, the CC will attend and participate in requested Family Team Staffing (FTS) for counties of responsibility and provide follow up care until youth is enrolled in residential facility.
- In cases of youth served, if residential treatment is recommended per the Family Team Staffing, the CC will work actively in securing residential placement, documenting all contact with residential facilities referral has been sent and follow up contacts until placement is secured.
- Engages with the individual and assesses natural supports already established. The care coordinator actively seeks out and encourages the full participation of the individual’s networks of interpersonal and community relationships.
- Determines what services are needed to strengthen the family unit and identify what barriers to care may exist.
- Advocates for the best interest and wishes of the client among the many service providers, agencies, family members and significant others.
- Helps families and individuals address and work through challenges when making life choices.
- Collaborates with insurance providers and community behavioral health agencies to ensure identified behavioral health services are made available.
- Models and reinforces responsible interpersonal and self-respecting behavior.
- Provides transportation as needed to obtain treatment plan goals and respite when applicable.
- Assists families and individuals with establishing a natural support system that provides ongoing support during and post program participation.
- Provides frequent contact for the first 30 days of services, ranging from daily to a minimum of three times per week. At least one face-to-face contact per week is required, other contact may include telephone contact and telehealth.
- Works independently seeking out and taking on new duties; consults with subject matter experts and supervisor to enhance knowledge.
- The master support plan of care is developed within two weeks of the initial support plan of care and reviewed no more than every 45 days.
- Adhering to the wraparound model, determine what services are needed to strengthen the family unit in an effort to promote stability in the community.
- Helps families understand developmental readiness and use teachable moments to surface issues that are important to helping them identify and reach their long-range vision.
- Uses information available from client families and other members of the system of care to develop the support plan of care, while using SMART principles (Specific, Measurable, Attainable, Relevant, Time bound), as evidenced by documentation in the progress notes and signatures on the service plan. Signatures of the person served as well as the parent / guardian are included.
- Ensures individualized goals match identified problems.
- Recognizes changes to the client situations and updates the support plan of care accordingly.
- Copy of the care plan is provided to the person served and members of the system of care.

Work Location: In the community work with travel within the County
Work Hours: Monday – Friday; 8:00 a.m. – 5:00 p.m.
Wage: The wage for this position is $20.00 per hour.
