Qualifications:

MINIMUM REQUIREMENTS: Bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development) and have a minimum of one year of full time or equivalent experience working with children with serious emotional disturbances; OR a bachelor’s degree from an accredited university or college and three years full time or equivalent experience working with children with serious emotional disturbances. Applicant must be able to obtain or currently have Florida Certification Board Credentialing as a Targeted Case Manager for both Adults and Children. If applicant does not have certification, the applicant must obtain certification within six (6) months of hire. Please include a resume with the application.  Applicant must be able to pass a Level II DCF criminal background check and drug screen.  Valid driver’s license with no major violations.   

POSITION SUMMARY: Knowledge and skills in community based behavioral health care case management, good interpersonal and communication skills, ability to demonstrate competent use of electronic health record and concurrent documentation. Targeted Case Management consists of services that help individuals link to needed medical, social, education, psychiatric, psychological, and other services. This includes direct assistance in gaining access to services, coordination of care, oversight of the entire case, and linkage to appropriate services. Case management includes (1) assessment of the eligible client to determine service needs; (2) development of an individualized integrated service plan; (3) referral and related activities to help the client obtain needed services; (4) monitoring and follow-up; and (5) evaluation. Educating persons served and their support system regarding diagnosis, treatment, and recovery.

Case Management services are provided in the community, in the client’s home, and in the office. Case Management does not include direct delivery of medical, clinical, or other direct services.
