MINIMUM REQUIREMENTS: Applicant must be able to obtain or currently have Florida Certification Board Credentialing as a Targeted Case Manager for both Adults and Children. If applicant does not have certification, the applicant must obtain certification within six (6) months of hire. Bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development). One year of full time or equivalent experience working with adults and children with serious emotional disturbances preferred. Please include a resume with the application.  Applicant must be able to pass a Level II DCF criminal background check, drug screening, and a Tuberculosis (TB) test.  Applicant must have a valid driver’s license with no major violations.

The E-MAT Case Manager will be expected to respond to emergency / crisis situations at the Fort Walton Beach Medical Center (FWBMC) Emergency Room, Okaloosa Schools, or with Law Enforcement (LE) in the field anywhere in Okaloosa County to prevent mental health or substance use hospitalizations.

The E-MAT Case Manager must have knowledge and skills in crisis intervention for both mental health and substance use, trauma informed care, safety planning, community based behavioral health care case management, good interpersonal coordination skills, time management, and communication skills, ability to demonstrate competent use of electronic health record and concurrent documentation. E-MAT Targeted Case Management consists of services that will link individuals to needed medical, social, education, psychiatric, psychological, and other services. This includes direct assistance in gaining access to services, coordination of care, and linkage to appropriate services. Case management includes (1) assessment of the eligible client to determine service needs to reduce the crisis and prevent hospitalization; (2) referral and related activities to help the client obtain needed services and engage support systems; (3) monitoring and follow-up until warm transfer to ongoing case management is completed; and (4) evaluation of services needed. Educating persons served and their support system regarding diagnosis, treatment, and recovery is also required.

Case Management services are provided in the community, in the client’s home, and in the office. Case Management does not include direct delivery of medical, clinical, or other direct services.
